Caricom Special Visa

Instruction Sheet For Application Form

The CARICOM Special Visa application form can be filled out online (recommended), printed, signed and submitted with the relevant documents. If you cannot download an application form, please contact the nearest visa issuing site, or any of the overseas Missions of the following CARICOM countries and request that the form be forwarded to you.

The CARICOM Special Visa application form is also available from any of the

United Kingdom’s Missions abroad.

The form is designed to be printed on plain letter size paper. Please ensure that the entire form is printed.

The form should be completed online before downloading or otherwise clearly completed in block letters in blue or black ink. Applications for visas may not be submitted online since other documents need to be forwarded with your completed application.

The following documents are required to complete your application:

1. VALID PASSPORT

The expiration date must exceed that of the visa application date by at least six months. Your passport must be submitted with your application form and must have a blank page on which to affix the visa.

2. PASSPORT PHOTOGRAPH

The photograph accompanying each visa application must be a standard size passport photograph i.e. 45mm high x 35mm wide, printed on low gloss, plain – white photo quality paper showing close up of your head and top of your shoulders in sharp focus and clear. Please ensure that the applicant's name is written in block letters at the back of the photograph. This is necessary if, for any reason, the photograph becomes detached from the application form.

Photo Appearance - The Photograph must:

1. be no more than 6 months old

2. show the applicant alone, looking directly at the camera with a neutral expression and his/her mouth closed

3. show the applicant's eyes open and clearly visible

4. exclude a head covering, except for religious reasons but your facial features from bottom of chin to top of forehead and both edges of your face must be clearly shown

The photograph may be in color or black and white, but must have a white background. The photo must be pasted on, preferably using double sticky tape or glue. (No staples or paper clips). For further details please click here:

3. APPLICATION FEE: PAYMENT OPTIONS

The application fee is $100.00 U.S.D net (non-refundable) which can be paid by credit card or Banker’s cheque (International) payable to IMPACS. Banker's cheque must be forwarded to the Issuing Site along with your application.

Children twelve (12) and under are not required to pay the visa application fee.

Cash or personal cheques will not be accepted.

Payment By Credit Cards

Credit Card payments via international Visa, MasterCard or American Express must be accompanied by a valid email address. No other credit cards will be accepted.

Applicants paying by credit card should note that once an order form has been submitted the credit card will be charged immediately. The card holder should ensure that there are sufficient funds to effect their requested transactions.

Your credit card payment transaction code number would be emailed to you for enclosure with your CARICOM Special Visa application as proof of payment. IMPACS will also notify all Issuing Sites of credit card payments made. If paying by credit card, please click here

Personal information collected by CARICOM IMPACS is held solely for the purpose of issuing the CARICOM Special Visa and CARICOM IMPACS will not use the information for any other purpose unless required to do so by law.

4. INFORMATION FOR YOUR GUIDANCE

Courier Service

The completed application form, passport and supporting documents are to be forwarded by courier service to the nearest issuing site. You must pre-pay the courier service for the return of your passport.

5. CHECK LIST

	Did you complete all the boxes in the visa application form?
	Did you sign the form within box?
	Did you attach a passport photograph?
	Did you enclose your valid passport?
	Did you enclose payment in the form of a Bankers cheque (international) or proof of payment via credit card?
	Did you submit a separate complete application for dependents travelling with you?
	Did you pre-pay with a courier service for the return of your passport?
	Did you print your name at the back of the photograph?

The submission of an application form for the CARICOM Special Visa indicates agreement with the terms and conditions of the application.

TERMS AND CONDITIONS

I understand that the expression CARICOM Single Domestic Space is limited to the following countries; Antigua & Barbuda, Barbados, Dominica, Grenada, Guyana, Jamaica, St. Kitts & Nevis, Saint Lucia, St. Vincent & The Grenadines, and Trinidad & Tobago, and not the other member states of CARICOM.

I am aware of and consent to the following: any of my personal data which appears on my application form for a CARICOM Special Visa will be supplied to relevant authorities within CARICOM IMPACS and the CARICOM Single Domestic Space, and processed by those authorities, if necessary, for the purposes of a decision on my visa application.

Such data may be input into, and stored in, databases accessible to the relevant authorities in the various states in the CARICOM Single Domestic Space.

I undertake to leave the territory of the states within the CARICOM Single Domestic Space upon the expiry of the visa, if granted.

I am informed that possession of a visa is only one of the prerequisites needed for entry into the CARICOM Single Domestic Space.
